Bar your first finger with your fretting hand on the 4th fret and hammer and pull-off with your right

C#|----------h12p4-h9p4-h7-(hammer with your fretting pinky)------------------|
G#|---------------------------------------------------------------------------|

this is repeated 4 times but at the end of the 4th time you add:
D#|---------------------------------------------------------------------------|
A#|---------------------------------------------------------------------------|
F#|---------------------------------------------------------------------------|
C#|------h9p4h7-h9p4h7--------------------------------------------------------|
G#|---------------------------------------------------------------------------|
C#|---------------------------------------------------------------------------|

Tone:
You can get the tone by using a fender-clean type tone with your guitar on the neck pickup.
