Chords: since I don't know the name of all the D/G/C/A/E variations, I'll just
number them my way, sorry to be so noobish. Corrections are welcome in the comments!

Intro: not easy to transcribe the rythm so listen to the song, roughly, the pattern goes
like:

down-down-up | up-down-up | up-down-up | up-down-up-down | up-down-up-down | up-down

So for the Chord diagramD MajorD part of the intro you go like this. The Chord diagramG+G part is basically the same:

It's hard for me to play and count how many ups and downs, so it might not be exact.
